Message from the Health Office concerning immunizations and physicals
Immunizations & Physicals NEEDED ASAP
Just a quick reminder that proof of immunizations and updated physical forms were due to the school nurse in September.  This NYS requirement is effective regardless of whether your student is participating in the remote or in-person learning this fall.   If the required health information is not received by October 9th your child may be excluded from in-person and remote learning.  Specific details for both requirements are provided below.

 Immunization Requirement

Per the NYSDOH School Assessment and Compliance Unit:
At this time, the NYSDOH is not issuing exceptions for immunizations because of COVID-19. The Department's stance continues to be that students missing vaccine doses for any reason must comply with Public Health Law Section 2164 requirements. Children entering or attending school in New York State, including summer school and distance learning, must comply with said requirements

NYS Physical Exam Requirement

NYS requires a physical exam for new entrants and students in grades pre-K or K, 1, 3, 5, 7, 9, and 11. NYS also required physical exams annually for interscholastic sports, working papers, and as required by the Committee on Special Education (CSE). The date of examination must not be more than 12 months prior to the start of the school year and noted on the form.
